Overview UnityPoint Health-Trinity Robert Young Center and CADS 1.0 FTR Care Coordinator Lead 40 hours/week Under the direct supervision of the Director of Integration the care coordinator will provide comprehensive care coordination services for adults Responsibilities Lead/Principle Duties and Responsibilities . Patient Flow/Work Process Assigns and check work per shift/day to established benchmark Identifies and recommends improvement opportunities. B Resource and Revenue Enhancement Use. Assigns staff per established benchmarks Assures charges are entered per established department standards * Identifies and resolves issues within scope of responsibility or Involves Manager/ Director. Satisfaction- Patient, Physician, Work Environment Identifies and resolves process or service problems/issues on a real time basis. Models behavior for meeting/exceeding performance targets Implements organizational strategies to achieve desired customer Service outcomes. Identifies, recommends and implements initiatives to improve satisfaction. Department/Service Quality Indicators Assures performance is consistent with established quality targets Identifies and resolves quality problems/issues real time Identifies recommends and implements quality improvement initiatives Safety- Patient Models behaviors for meeting/exceeding safety performance targets Identifies and resolves safety problems/issues on a real time basis. Identifies recommends and implements safety improvement initiatives. Provides input to manager/director for employee Identifies variance performance expectation and provides feedback to employee manager. Principle Duties and Responsibilities Provides assertive community outreach and sustained efforts to engage individual clients. Collects information necessary to develop an accurate assessment of each client, including completion of the Multnomah Community Assessment Scales (MCAS) Provides emergency assessments of persons with mental illness in the community. Accurately assesses patients for suicidal gestures and ideation. Assists in the timely development of the initial treatment (IT) plan and monitors the client's progress toward achieving their goals. Provides linkage to other mental health services and programs including psychiatric services, medication management, representative payee Assists the client in securing basic necessities ( e.g. food, housing income, medical care) Provides counseling, problem-solving and personal support. Engages the client's family and personal support systems. Teaches and assists with activities of daily living in home and in the community. Uses cognitive rehabilitation, in-vivo skills practice and role rehearsal (psychosocial rehabilitation). Completes all necessary documentation in a timely and accurate manner and in compliance with established standards of the State of Illinois and the Robert Young Center. Accurately enters all charges within 24 hours of delivering the service. Provides disease management and holistic patient care. Assists clients in the development of crisis plans. Will assist clients in the development of a Wellness Recovery Action Plan. Qualifications Requires a BA, BS, BSW in behavioral healthcare services related field, or a practical nurse (LPN) licensed under the Nursing and Advanced Practice Nursing Act (225 ILCS 65) with 2 years supervised experience in mental health or human services, or a certificate of psychiatric rehabilitation from a DHS approved program plus a HS diploma and 2 years experience in providing mental health or human services, a recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, plus one year experience providing mental health services, or minimum of five (5) years supervised experience in mental health or human services and a HS diploma.
