Pay Information $2,454 per week About The Position Job Title: RN Case Management Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: 2 years of large hospital inpatient Case Management experience required License: Current RN license Certifications: Case Management certification preferred, BLS - American Heart Must-Have: Must be proficient in care coordination. Must have experience in discharge planning. Must possess skills in assessment of psychosocial needs and support. Must have previous experience providing care coordination and discharge planning. Must use a personal cell phone for communication. Must understand that this is a drug-free environment and be able to produce a negative drug screen. Must have a BSN. Description: This position involves assessing patients for both discharge planning and psychosocial needs. You will work with an interdisciplinary team to develop a plan of care for transition to the next level. Overall care coordination for a team of patients will be provided. Experience with care coordination and discharge planning is essential. Familiarity with relevant software is a plus. Candidates may be required to float to different units of care as necessary. The expected nurse to patient ratio is not applicable for this role. Dress code is business casual, with comfortable walking shoes and a white lab coat required. Training will include navigating the care management model and identifying patients in need of management and care coordination.
