Description The Care Manager, Telephonic Behavioral Health 2 , in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. Responsibilities The Care Manager, Telephonic Behavioral Health 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures. Required Qualifications + Licensed Masters level trained Behavioral Health Clinician (e.g. LMSW, LCSW, LPC, LMHC) or licensed RN with Psychiatric experience + 3+ years of care management experience which includes at least 2 years' experience in an integrated care setting managing members with both Medical and Behavioral Health needs + 3+ years of work experience with members who experience Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD) + Minimum of three years of experience in behavioral health direct patient care Preferred Qualifications + Knowledge of community mental health and social service agencies and additional community resources + Bilingual + CCM certified + Experience with multiple physical and mental diagnoses within the geriatric population Additional Information Be a part of our Clinical Space - engage our members and guide them to develop lifelong wellbeing and health. Humana is seeking an IMBH Care Manager to telephonically assess, evaluate, and support members who are challenged by both serious mental illness and complex medical situations. Our goal is to provide support for both mind and body to help each member attain their optimal health by addressing behavioral health needs so that our members can better care for their physical health. Apply your clinical knowledge to educate and empower your members, to take charge of their own healthcare decisions. Provide guidance and clinical expertise on how to navigate through the health care setting. Provide ongoing consultation with your members via telephone to ensure that they are receiving the best quality of care possible. You will guide members and their families toward, and facilitate interaction with, resources appropriate for their care and wellbeing. You will work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches, and techniques to manage a member's physical, emotional, environmental, and psycho-social health issues. Other responsibilities include: + Proactive telephonic outreach to eligible Humana members per Humana at Home service level guidelines + Complete physical, psychological, emotional, and environmental assessments, for the purpose of providing appropriate, timely interventions to ensure the provision of optimal care + Develop a plan of care with your member based on identified needs and recommend, and/or coordinate, interventions that may include information, education, and removal of barriers to close gaps in care + Coordinate community care and services as deemed appropriate + Work collaboratively with other members of the Multi-disciplinary team - to include; Social Services, Community Health Educators, Dieticians, Pharmacists, Psychiatrists, Utilization Management teams, and others as appropriate + Comply with performance and reporting standards as defined by Humana Corporation + Utilize Motivational Interviewing and Solution-Oriented approaches in communication with members + Adhere to compliance standards, state and federal regulations and individual licensing boards + Participate in professional development activities + Additional responsibilities as deemed appropriate by Humana leadership Technology + Intermediate to advanced computer skills and experience with Microsoft word and excel + Strong computer, web navigation and research skills + Ability to learn how to use virtual information systems (example would be WebMD) + Excellent keyboard and web navigation skills + Ability to use a variety of electronic information applications/software programs Work Requirements + Ability to work a minimum of 40 hours per week Monday-Friday between the hours of 8am- 8pm EST (flexibility sometimes required to meet member call time needs) + Ability to interact effectively with multi-disciplinary team members in virtual environment + Behavioral Health Licensed Care Managers must be licensed in the state they live + Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work + Must have accessibility to high speed DSL or cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance from Humana systems is 10M x 1M + Associates working in the state of Arizona must comply with the Tobacco Free Hiring Policy (see details below under Additional Information) and upon offer will be subjected to nicotine testing as part of a 10-panel drug test Scheduled Weekly Hours 40 About Us Mission: At Humana, our cultural foundation is aligned to helping members achieve their best health by delivering personalized, simplified, whole-person healthcare experiences.
